Introduction

The project targets to reduce the development of some of the common preventable health conditions through the creation of employment opportunities to the less advantaged individuals in the community. The preventable health condition includes respiratory diseases, cancers, and cardiovascular diseases, which are brought about by smoking and alcohol consumption (Stanley, 2017). The project serves to target the cause of the health conditions rather than the approach used by the government and other organizations of treating the condition themselves. It is therefore considered feasible as it not only reduces the preventable health condition but also reduces the cost incurred on healthcare delivery and promotes the economic growth of the country. The approach as well improves the quality of living of the individuals in the community (Adams, 2018). The project shall be integrated along with the services offered at Ascension community centers. There will be the identification of the less advantaged individuals in the community through the use of the nurses. Once the less advantaged individuals are identified, they are assigned jobs that they are competent to do, which are safe as well. Then over the period, there shall be monitoring of the health status of the people in the community to determine the changes the program has brought in the community. It is postulated that the project shall cost about $55,877,850 to run and ensure there is sustainability. The benefits harnessed from the project are far much superior to the cost invested in because it not only benefits the individual, it translates positively into the community by promoting the health community and reduces the cost spent seeking medical care.

Project description

The project aims at the creation of job opportunities for the people in the communities that are less advantaged to improve their health standards. The approach comes as an attempt of the project to reduce if not eliminates the number of new cases of individuals developing chronic illness. As demonstrated, the approach of creating employment to the people is far much superior because it benefits the community and, at the same time, brings a positive impact to the government and the non-governmental organization (Adams, 2018). The Ascension community center, which is the non-governmental organization, promotes the health of the people through building the centers that help the patient manage their chronic conditions. The project aims to be integrated into the Ascension community center to reach the people in the community and offer the employment services which are targeted to help reduce the development of preventable illnesses among those who are unemployed.

The project is driven by the desire to cut the rate of the preventable chronic condition by 50%. Through the employment opportunities give to the people in the community, they can cater to the appropriate health, eat well, and thus prevent the majority of the diseases. Studies have shown that the lack of finances leads to stress that drives the people in indulging in life-threatening activities such as alcohol misuse and tobacco smoking which negatively impact on the health.

The project is the target to integrate the government and the necessary authority to ensure that the project is useful. The government plays a crucial role in affecting job availability as well as enacting the employment laws which should be upheld by the authority. In the running of the project, the nurses are identified to be a vital component in connecting with the community. These shall help in determining the various skills of the people in the community, which will help in recruitment. Similarly, the project shall promote the advancement in education, leading to the building of skills towards employment. Therefore, the core purpose of the project shall be reducing the rate of preventable health conditions by offering job opportunities to those unemployed. This will be done in conjunction with the Ascension community center.

The project shall be done in four phases that include the initiation phase, planning phase, implementation phase, and evaluation and monitoring phase. This is aimed at ensuring that the activities are handled in each phase appropriately without leaving any gap that could lead to inefficiency in the project.

Initiation phase: During this phase, the project shall be presented to the Ascension Community organizational management, the government, the local authority, and the community. The project involves the creation of employment, which will help alleviate the chances of development of the common preventable health conditions. Its benefits shall be laid down to all the four parties so that it can be embraced. The project is seen to reduce the development of preventable health conditions due to smoking and alcohol use and, at the same time, minimizes the healthcare cost to the government, promote quality of life among the people, and improve the growth of the country. It is essential to introduce the plan to the government because it forms the basis of employment laws that would affect the project. Informing the community help create awareness and promote community participation.

Planning phase: It shall entail the identification of the nursing personnel who shall take the lead in the project. The nurses shall be the contact with the community and help in data collection, which will be necessary for recruitment and monitoring. The phase shall involve training of the nursing personnel to ensure they understand the project. It as well helps promote efficiency at work. Identification of the centers to be used shall help in resource allocation that translates into the sustainability of the project. Identification of an individual’s abilities and skills in this phase forms a precursor in determining the nature of employment that best suits the given individual in the community.

Implementation phase: It involves taking the step of making the project work out. It consists of the recruitment of the individuals of the community to jobs they are capable of doing. This shall help promote the willingness to undertake in the process at the same time assisting in the generation of finances that help improve health status. Recruitment of the nursing staff caters to the adequacy of the necessary workforce to ensure the efficiency of the project.

Evaluation and monitoring phase: It is in place to determine the effectiveness of the project. Through the data collected from the individuals following their recruitment to various jobs, the health status can be compared to determine whether there is an improvement or not. The phase shall involve the monitoring of the people by doing regular checkups to assess the development of any preventable health condition. Description of work

External consultants- The external consultants shall help in guiding the new adventure that involves the incorporation of the healthcare services with the recruitment of the people from the community. The external consultants from different areas of specialization will shine in brighter lights in matters concerning the hiring of the employee, the criteria for valid matching of the people in the community to their abilities and skills. The input shall enable the people in the employee to be enrolled in the jobs they have an understanding of, thus increasing the willingness to partake in the offered job opportunities. Not to forget that the external consultants as well are potential employers to some of the people in the community based on the abilities and skills that one possesses.

 

Top executives of Ascension- The senior executive members of the Ascension Community center play an essential role in the project. They are responsible for giving the go-ahead of the integration of the healthcare services with the plans to evaluate and assign the locals various job opportunities. Since the nurses to the organization shall be spearheading the process, the top executive shall help in enumerating some of the nursing personnel to be used in the processes. The senior executive shall help link up with local authority in the initiation of the program to the community. Therefore, their role span from the initial stages of the project until the end.

 

Community leaders- The community leaders serve as the local authority at various points where the community centers are situated. Community leaders play as well an essential role in terms of promoting the project within the different locality within which they are located. When the community leaders embrace the concept, the penetration of the project to the community becomes more comfortable and practical, leading to many people benefiting. Human resource management- The human resource management department of the Ascension community center shall help in the implementation of the project strategies to ensure that the project is not only initiated but sustained to promote desired outcomes. The office shall assist in the recruitment of the identified individuals in the community with the desired skills and abilities to various job ventures created. Similarly, the office shall enable the distribution of resources to the different sectors to be able to promote the running activities. These resources can be in the form of personnel as well as finances that would help in the running of the project. The sustainability of the project highly depends on the availability of necessary resources, which makes the office important in promoting the efficiency of the project. The human resource management department shall as well help in the recruitment of the new staff in an attempt to ensure the adequacy of staffing when this project is incorporated into the organization. It shall carry out the training session to equip the staff competent of the project, thus enhancing the efficiency. It is, therefore, a crucial sector as far as the project is concerned.

 

Community members- These are the target people in the project. Their role as far as the project is concerned is to participate in the recruitment process for the various jobs offered and thus enable them improves the quality of living. The project targets to improve the living standards of the people who are less advantaged so that they may avoid engaging in unhealthy practices such as smoking and alcohol use, which predisposes to common chronic health conditions. Therefore in the project, they are the participants who should be the center of improving their health status through employment.

The Ascension’s community services project that aims at creating quality and long term jobs for the less advantaged communities served by Ascension is focused on helping the community members to have a way of leading healthy lives in the long term. The nurses are seen to be the vital components in ensuring that the program runs effectively. Through identification and assigning the individuals in the community the job opportunities permanently, they can sustain themselves and improve the quality of life, which reduces the chances of developing some of the preventable health conditions. Continuous monitoring and evaluation of the process help determine the effectiveness of the project. It ensures that the project remains sustainable and attains its objective.
